Enrollment Forms
Thank you for choosing Van Nuys Middle School for your child's education!
What do you need to enroll your child?
1. Proof of child's age (birth certificate, baptismal certificate, passport, etc.)
2. Immunization Records (All 7th-grade students need their TDAP)
3. Proof of address, verified by a utility bill (Dept. of Water and Power or Gas Company).
Below, you will find the necessary forms to enroll your child. Please download the forms, fill them out, and bring them to the Attendance Office. We take open enrollment applications!

3. Media Waiver Form
Please click here to complete the Household Income Form (HIF), which entitles our school to receive additional Federal and State monies.
